PERITONEAL MESOTHELIOMA
Peritoneal mesothelioma, a cancer of the lining of the abdominal cavity, is less common than the pleural form, comprising approximately one-fifth to one-third of the total number of mesothelioma cases diagnosed. According to the SEER (Surveillance, Epidemiology, and End Results) database, these diagnoses are approximately 54.7 per cent male versus 45.3 per cent female, with the median age being 65-69. The latency period appears to be shorter for asbestos-exposed individuals with symptoms appearing 20-30 years after exposure rather than the 30-40 year latency more commonly associated with pleural mesothelioma.
Symptoms
Clinical symptoms at the time of presentation may include abdominal pain, abdominal mass, increased abdominal girth, distention of the abdomen, ascites (fluid in the abdomen), fever, weight loss, fatigue, anemia and digestive disturbances. Some patients complain of more non-specific symptoms for a number of months prior to a confirmed diagnosis. In a percentage of cases, peritoneal mesothelioma is found incidentally when the patient has sought help for another health problem such as gallbladder, hernia or pelvic mass.
Diagnosis
As with all mesotheliomas, the diagnosis of peritoneal mesothelioma can be challenging. CT findings may help differentiate between the two clinical types of peritoneal mesothelioma, termed “dry” or “wet”, since their appearances are very different upon imaging. In the “dry” type, CT may reveal multiple small masses or a single dominant localized mass. There is normally little or no ascites. In the “wet” type, CT may reveal widespread small nodules, but no dominant mass. Ascites is usually present.
If fluid is present, it may be removed in a procedure called paracentesis. Unfortunately, as is the case with pleural mesothelioma, fluid analysis offers limited diagnostic value. It is normally a tissue biopsy obtained in a laproscopic exploratory that will yield a definitive diagnosis.
Staging
There is currently no established staging system for peritoneal mesothelioma, and if the disease is staged, it is normally done in accordance with the TNM system, the most common general cancer staging system. This system refers to the status of the tumor (T), lymph nodes (N) and metastases (M). There are general categories which may also be somewhat helpful in determining stage.
The first category shows a localized lesion able to be completely resected (entirely removed). In the second category, the disease is contained within the abdominal cavity on peritoneal and organ surfaces where debulking (the removal of as much, but not all of the tumor) is possible. Category three shows disease contained within the abdominal cavity with invasion of organs such as the colon or liver. Category four shows disease extending outside the abdominal cavity.
Treatment
In recent years, multimodality treatment of peritoneal mesothelioma has become more common for a select patient population, since surgery alone and/or intraperitoneal chemotherapy alone have proven to be similarly ineffective. Cytoreductive (debulking) surgery involves the removal of all or nearly all visible tumor, and, depending on the physician’s choice, may be combined with Intra-Peritoneal Hyperthermic Chemotherapy (IPHC), intraperitoneal chemotherapy and/or radiation. Dr. Paul Sugarbaker has written "intraperitoneal chemotherapy gives high response rates within the abdomen because the peritoneal space to plasma barrier provides dose intensive therapy." Since it is not always possible to remove all tumors, the prognosis for long-term survival may be based on the completeness of cytoreduction.

You are never too old to learn. Don't let learning stop just because you're out of school. Go to your local library to gain more knowledge. It is a great place to relax, gather thoughts, and focus on studying. If you have any spare time, carry a book over to the park or stop in at a family restaurant. It all aids in building a sharper better mind, and improves your attitude.
You should treat yourself as well as you treat others. It will improve the way you think. You will be a happier individual with a sharper mind and better attitude. Try buying yourself a small gift while out shopping for others. It is always good to give, and being generous is nice, but charity should begin at home. Remember, too, that generosity involves more than just material things and satisfaction is not achieved by consumption.
Figure out for yourself the true meaning of what is important in life. In doing so you will build a sharper mind and a better attitude. For where others may have failed and so never know the true meaning, you may succeed. Search deep inside for your inner self.
You are truly blessed if you have common sense. There is nothing like good old common sense, for without it how can you possibly achieve having a sharp mind?
Use your mind more instead of using a calculator or adding machine. This is to say, especially, on simple things that you can easily total up in your head or on a piece of paper. Many people haven't done long division since grade school; give it a try sometime.
Stay focused on making your own decisions. Don't let other people try to make decisions for you. Be firm about speaking for yourself. And don't be afraid of expressing your opinion. Everyone has an opinion and the right to have one. Who knows, your opinion may catch on.
You should jog, run, ski, play ball, fish, write, garden, or whatever makes you happy. Doing things that you enjoy will help you to keep your mind fresh and clear and your attitude at its best.
Learn some basic memory techniques. To start off with, you could find some books written by Tony Buzan or Dominic O'Brien: both of them are top people in the memory world.

Bill Royall chose the University of Denver so he could take the winter quarters off from school and hit the slopes. His powder-searching ways continued after college, landing him in Vail for a few years before it eventually became too crowded, too trendy. A move to Aspen ended with the same claustrophobic result. And so it went, Royall's quest for a quiet skiing sanctuary going on for years, taking him to places like Steamboat Springs, Taos, Sun Valley. And it always ended the same: moving out when the crowds moved in.
The nomadic journey seemed to come to an end about 20 years ago when he arrived in Jackson, Wyo., a place still oozing with that dusty-floor saloon charm of the Old West. But, like all the other ski towns that had lured Royall with its charms, Jackson changed, attracting fuzzy-jacket-wearing out-of-towners to the slopes and trendy shops with pricey paintings and sparkly T-shirts lining the streets.
Then Royall found Grand Targhee. A down-home alternative to upscale Jackson Hole, Grand Targhee is a throwback to the days of the small-town ski resorts, the kind of place where lift operators know the regulars' names and the glistening snow is the only sparkle anyone will ever need. "When you go to a lot of big-time resorts, it's a lot of hustle and bustle, glitter and glamour and we don't want that," says Royall, a stone carver from Southport, Maine, who spends three or four months every winter in Grand Targhee with his wife, Emery. If that vista doesn't inspire love, tell your sweetheart it was the setting for U2's "Two Hearts Beat as One" video. And while it might make that engagement ring you bought dim in comparison, time your walk back so that you see the Eiffel Tower after nightfall: At the start of every hour until 2 a.m. (1 a.m in winter), its 20,000 bulbs glitter for 10 minutes. A lot of talk that this is the end for coach Craig MacTavish after his disastrous stick-measurement gambit against Anaheim went bad earlier this week. Only Lindy Ruff (Buffalo), Barry Trotz (Nashville) and Jacques Lemaire (Minnesota) have coached longer with their current teams, and MacTavish is a treat to work with from the media perspective. Do you think this is it, and, if so, who steps in? I think, barring a miracle finish, this is it. And wouldn't a guy like Tom Renney look good behind the Oilers bench?
Pierre LeBrun: I do indeed speak French and English, but it's the language of hockey that mattered Tuesday night when MacT gambled the season away on the stick call. Marty McSorley, where art thou? Funny thing is, if Teemu Selanne's stick ends up being illegal, the Oilers coach is a genius. Such is the fickle nature of our business. And I like the way MacTavish handled the fallout the next day -- took it like a man, answered questions about his future and didn't sound testy with the media at all. He's a real likeable character.
But will he last past this season? The new owner, Daryl Katz, apparently loves the guy (Dan Barnes of The Edmonton Journal reported Friday that Katz said MacT isn't going anywhere). Team president Kevin Lowe is very, very tight with MacTavish. But I still think the time has come. That's OK; MacT won't be without a job for too long, I can guarantee you that. Same goes for Renney, who I agree would be a great fit in Edmonton (if not Calgary, but I digress). But you touched on Ruff and his endurance in Buffalo. What would you do there?
Burnside: Talk about a team that is likewise done like dinner (sorry, Oilers and Sabres fans). Buffalo lost to Atlanta on Wednesday night, the second time it failed to defeat the Thrashers in the past couple of weeks; even though the Sabres earned a point in both cases, the points left on the table will haunt them through the summer.
I don't sense the same kind of unrest in Buffalo as there seems to be in Edmonton. The Sabres were without Thomas Vanek and Ryan Miller for key parts of the season, but I still don't think this team has found the leaders in the room to take them over the hump. You've mentioned it before: Craig Rivet comes over from San Jose and becomes the team's captain? Rivet is a terrific guy, but it doesn't say much for the group that has been in the Sabres' locker room the past three or four years and hasn't seen a leader step forward. What do you think, is it time for Ruff? Not to sound like Renney's agent or anything, but the Sabres are another team that would, I think, thrive under Renney's tutelage.
